Why We’re Asking for This Information 


To help protect your business and prevent caller ID fraud, the FCC requires voice service providers to verify the phone numbers a customer uses when placing outbound calls on the provider's network. 


Policy Overview


altafiber requires that all outbound calls transmitted across its network present a legitimate, accurate, and authorized CLID. To ensure compliance, customers must do the following: 

  1. Use only telephone numbers you own or are legally authorized to use. 
  2. Do not present numbers that are: 
    • Unallocated, invalid, or disconnected 
    •  Assigned as inbound-only or designated Do-Not-Originate (DNO) 
    • Assigned to another party without explicit written authorization
  3.  Provide updates before adding or changing any CLID values used on the network. 


altafiber may block or suspend outbound traffic using numbers that violate these policies or federal requirementsCompletion of this form is required. 


We will require an update of this form for the following account activities: 

  • Installing new SIP, ISDN PRI/BRI trunking or outbound voice service setups 
  • Adding or changing caller ID numbers 
  • Annual updates
